Digital Service Agreement
Our service contract is short and very easy to understand. In order to receive services from MuvDigital, we require that all of our clients sign our Digital Service Agreement. This service agreement protects both our clients and MuvDigital based on the terms of the contract. Initial Client Information Form
In order to better serve our new clients, we ask that they provide as much information about their business as possible. This information will be used in the web design process, management, and marketing of your business. Additionally, it will make the design process smoother.
Please provide the following:
- Copy of Logo
- Pictures Desired on Website
- Page Names & Content (Text)
- Social Media Links (Facebook, Twitter, LinkedIn, Pinterest)
- Hours of Operation
- Forms of Payments Accepted
- Downloadable Pages – For your customers to fill out
- Testimonials (If Desired)

Recurring Payment Authorization Form
In order to process your monthly payment or one-time payment, we’ll need your business checking or credit card information. This form is for both recurring payments and one-time payments. Additionally, this form grants us access to process payments that you request to be made.
